If your child is unable to participate in physical education classes, the health office must receive a note from home. A parent or guardian note is valid for a maximum of one week. For a period of time longer than one week, a doctors note including the diagnosis and the duration is required. In the case that your child needs to use assistive devices such as a cane, crutches, or a wheelchair, a doctor's order is needed.
