Required vaccines for entrance into Kindergarten through grade 12:

• 2 doses of measles and mumps vaccines and 1 dose of rubella vaccine (or 2 MMRs)
• 3 doses Hepatitis B vaccine

Additional required vaccines for entrance into Grades Kindergarten through Grade 5:

• 5 doses of diphtheria and tetanus toxoid-containing vaccine and acellular pertussis vaccine (DTaP)
  • If 4^th dose received after age 4 years, only 4 doses are required

Additional required vaccines for entrance into Grades Kindergarten, 1 &6:

• 4 doses of poliomyelitis vaccine (IPV)
  • If 3^rd dose received after age 4 years, only 3 doses are required
• 2 doses Varicella vaccine

Additional required vaccines for entrance into Grade 6:

• 1 doseTdap vaccine

Additional required vaccines for entrance into Grade 7 as of the beginning of 2016 school year:

• 1 meningococcal vaccine

Acceptable proof of documentation of  immunizations are:

• Health care practitioner record
  • Signed by practitioner licensed in New York State
• Records acceptable without a signature
  • NYSIIS or CIR record
  • Official registry record from another state
  • Electronic health record
  • Official record from a foreign nation
  • School health record

Medical exemptions from any immunization must be renewed by your physician every school year.

Please contact your primary care physician in order to discuss and obtain any immunizations your child might need in order to be able to start school on time in September.
